Sullivan Elementary Kindergarten registration

Boys - Wednesday, April 13th 1-6 p.m.

Girls - Thursday, April 14th 1-6 p.m.

In the Sullivan Elementary School Cafeteria

Please park in the sough parking lot and enter through the back doors.

Children must be 5 years old by Aug. 1st to enroll.

You must bring with you to enrollment:

• Your child
• Birth Certificate
• Immunization Records
• Social Security Card
• 2 Pieces of mail with you physical name and address - no P.O. Box

For your convenience, you may come to the SES office April 4th - April 12th and pick up an enrollment packet.  Office hours are Monday thru Friday 7:30 a.m. - 3 p.m.
